Primary health care and wellness services

The WBOP PHO supports and delivers primary health care and wellness services through general practices, iwi partners, and community organisations from Katikati to Ōpōtiki.

We do this by planning, funding, and coordinating services, projects, and programmes alongside providers. This helps us offer our wider WBOP community options to support their health and wellness needs.

Many of our services focus on specific diagnoses or illnesses. However, we also aim to support groups in our community who have been underserved in the past, such as Māori, the Disability community, Pacific peoples, and our Rainbow whānau. For these groups, we provide extra support systems around diagnosis and illness or offer services that meet their specific needs.

IPMHA - Integrated Primary Mental Health & Addictions

Do you need help with your mental health or physical health? You can find Health Improvement Practitioners (HIPs) and Health Coaches at your medical centre, these are free if you are enrolled with a practice.

Diabetes Support & Lifestyle Changes

Our team of community dietitians, nutritionists, and nurses can help you manage diabetes and make healthy lifestyle changes.

Quit smoking

Start your smoke-free life with our Stop Smoking programmes and personalised support. These programs are free and aim to give you the tools and support you need to successfully quit smoking.

School-Based Health Services

Speak to a healthcare professional (school nurse) during school hours! Use our free School-Based Health Service for students in years 9 to 13 at secondary school or alternative education facilities.

After Hours Care

For urgent but non-life-threatening issues, choose from in-person or online consultations to get the help you need.

Here are more services and programmes you can access through our general practices, community providers, and iwi partners. Some of these may need a referral from your doctor or meeting certain criteria.

Acute Demand Service: Avoid Hospital Visits

This programme gives you a faster and easier way to get urgent care, so you don’t have to go to the hospital or spend a lot of time in the Emergency Department. If you have a sudden health problem, like trouble breathing, your doctor will see if you qualify. Instead of going to the hospital for more tests, you will be sent to a primary care provider, like a radiologist, for diagnosis and treatment at no cost. Services include X-rays, ultrasounds, IV hydration therapy, cellulitis treatment, and sometimes free follow-up appointments.

Clinical Pharmacist Medicine Optimisation: Safe and Effective Medication

This service, provided by Medwise, helps make sure your medicines work well and are safe. Special pharmacists team up with your doctor to check, manage, and sometimes give out medicines. Medwise looks at how well you stick to your treatment plan, checks your medicines to avoid mistakes, and teaches you about your medications. You might get help from a Medwise pharmacist if you take several prescription medicines regularly, have recently been in hospital, or have had recent changes to your medicines.

Breast & Cervical Screening Support Service: Ensuring Access

Our outreach support service is here to help you get your breast and cervical screenings done. We offer a range of support, including transport to appointments and follow-ups, help with colposcopy, scheduling appointments, providing general nurse support and culturally sensitive care. This service is specifically designed for Māori, Pacific Peoples, CSC holders, and everyone overdue for screening as per criteria. Ask your doctor or community service provider to refer you to our service or call now 0800 920 033.

Coordinated Primary Mental Health Service: Personalised Support

We offer a range of free therapeutic services to support you, including social work, youth-focused group therapy, extended consultations with GPs or nurses, and counselling for issues like depression, anxiety, alcohol/drug concerns, and youth mental health issues. These services follow a 'Stepped Care' approach, which means we start with the most suitable and least invasive treatment after talking to you about your needs and the options for support. If more support is needed, we move to more specialised services. This ensures you receive personalised care that matches your needs at every stage. Your doctor, nurse practitioner, school nurse, school counsellor, or Health Improvement Practitioner can refer you to this service.

COPD Management: Improve your Quality of Life

Our Chronic Obstructive Pulmonary Disease (COPD) programme offers four free visits each year. It helps you manage your condition better so you can have a better quality of life. The programme includes a flu vaccine every year, advice on quitting smoking, encouragement to stay active, and a personalised COPD Care Plan. We work on spotting symptoms early to avoid hospital visits and provide extra support if needed. To join, you need to be enrolled with WBOP PHO and have a confirmed COPD diagnosis by spirometry. Ask your doctor for a referral.
