Self-Management Groups
We offer regular Self-Management Groups (SMGs), one for diabetes and one for lifestyle change, which are available both in person and over Zoom.
These free, interactive group sessions are a great opportunity to learn how to self-manage your health while connecting you with other people living with Type 2 diabetes or wanting to make positive lifestyle changes.
The sessions are led by a registered dietitian or nutritionist, with support from a diabetes nurse, pharmacist, Sport Bay of Plenty and the Diabetes NZ Tauranga branch.
Options:
- Morning/Afternoon (2 weeks)
- Evenings (5 weeks)
Groups run in Tauranga South, Mount Maunganui, Te Puke, Pāpāmoa, Katikati, and via Zoom.
You can self-refer or ask your GP, nurse, or HIP to refer you.
Free individual diabetes consults
We also offer 1-to-1 individual consultations for patients requiring support with their diet. We can see you in the clinic or provide consultations online. If needed, we can arrange a home visit.
To receive this service, you must meet the criteria and have a referral from a medical health professional.
What is a Self-Management Group (SMG) for diabetes?
An SMG for diabetes is a group led by a registered nutritionist or dietitian and supported by a diabetes nurse, pharmacist, Sport Bay of Plenty and Diabetes NZ.
In these groups you learn about what diabetes is, the value of physical activity, healthy eating while diabetic, types of medications and their role in your treatment, complications that can arise with diabetes, travel tips, the effect of stress and sleep on diabetes, plus the value of goal setting and self-management.
We offer the SMGs both in the morning and afternoon over two weeks OR evening courses over five weeks via Zoom. The physical sessions are available in Tauranga South, Mount Maunganui, Te Puke, Pāpāmoa, and Katikati. Each group is 5-10 people, and you are welcome to bring along a person to support you.
What is a Self-Management Group (SMG) for Lifestyle Change?
An SMG for Lifestyle Change is a group led by a registered Nutritionist or Dietitian and focused on giving you the tools to make positive changes toward a healthier lifestyle.
In these groups you will learn about common health conditions and the tests to diagnose them, physical activity, healthy eating, mindful eating, stress and sleep, as well as goal setting and self-management.
The SMGs are offered both morning and afternoon over two weeks OR evening courses over four weeks via Zoom. The physical sessions are available in Tauranga South, Mount Maunganui, Te Puke, Pāpāmoa, and Katikati. Each group is 5-10 people, and you are welcome to bring along a person to support you.

What is the Dietetic Service?
Do you need help with special dietary needs?
Our Dietetic service provides 1-to-1 consultations. You can meet with us in person at the clinic, or online through telehealth. If needed, we can also arrange a home visit.
There are capacity limitations for 1-to-1 consultations, so we encourage this service only for those who are not appropriate for our self-management groups. For example, patients with hearing loss, who require a translator, experience severe anxieties in group environments, or have intellectual disabilities. Otherwise, please refer to our self-management groups.
